Vanderbilt Travel Risk Assessment Committee (VTRAC) - Faculty/Staff International Travel Request

Section 1

Indicate the country of citizenship for the passport you intend to travel on.
List the country/s, city/s, and arrival date/s for each destination included on your proposed itinerary.

Section 2

Please complete the following items to provide the Vanderbilt Travel Risk Assessment Committee (VTRAC) with pertinent details related to your travel plans and itinerary. The completed plan will be sent to the VTRAC for review. Please note that review may take up to 3 weeks. If you need assistance or have questions about completing the safety plan, please contact for assistance.
This should include a brief description of your research or travel purpose. Please list any additional Vanderbilt faculty, staff, or students who will also be traveling with you.
Please list any family members or dependents who will be accompanying you during travel. View the Travel and Business Expense Policy and Human Resources Vanderbilt International and Domestic Travel FAQ for more information about travel companions.

Health, Safety and Security Risks

Confirmation that you understand the safety, security and/or health risks involved in travel to your destination/s.

Have you identified the address of the nearest U.S. Embassy or Consulate for each destination? 

Have you enrolled in the US Department of State Smart Traveler Enrollment Program (STEP), or your home country equivalent for each destination.  

Have you registered your travel with Vanderbilt University? If you have not booked travel in Concur, please download and complete this document and upload it as supporting documentation at the end of this form.

Have you enrolled in Vanderbilt’s GeoBlue international health insurance?

Are you aware of Vanderbilt Incident Response for Travel (VIRT) and how to use VIRT in the event of an emergency?

Communication Plan

An explanation of how you will be contacted while you are traveling, including in-country contact information if going abroad.

If the number is available at this time, please provide the phone number (include country and city/area code). If applicable, please describe how and when you will obtain a working cell phone. Please also list other ways you can be contacted, such as social media (e.g. secondary email, Whats App, Viber, Facebook name, Twitter, Instagram, etc.)

Itinerary and Logistics

If you have booked your travel outside of Concur or not using World Travel services, please include a copy of your itinerary if you have it already. You can type in the box below or upload a file. 

If you booked in Concur or with World Travel services, your itinerary is automatically entered into our security provider portal.

Naming Convention: YYYYMM_Faculty Member Last Name_Destination_Itinerary. Use the year and month of intended travel start date.

The GSS team provides health and safety guidance to all VU travelers. They can assist with individualized health and safety advice, such as traveling with medication, setting up continued care, using Vanderbilt-issued international health insurance, addressing identity-based safety and health concerns, and more.
